You may have noticed leaflets and posters around the surgery informing you of NHS England's care.data.
This is a new scheme were information about you and the care you receive is shared (in a secure system), by healthcare staff to support your treatment and care. The NHS wants to use this information to plan and improve services for all patients, by linking information from all the different places where you receive care (ie GP, hospital and the community). This will provide the NHS with a full picture to compare the care you received in one area against the care you received in another.
Your postcode and NHS number, but not your name, will be used to link your records in a secure system so your identity is protected. This information can then be used by others such as researchers and those planning health services, to make sure we provide the best care possible for everyone. This is all controlled by strict laws and rules to protect your privacy.

It is important to realise that if you do not agree with the programme and you do not wish your information to be shared, then you need to opt out of the programme. The Opt Out forms are available on reception, or can be downloaded below. Please hand into reception once complete.
