How to contact us

The online form is the quickest and simplest way for most of our patients to access the clinical team for assistance.

Our phone lines remain open for anyone with accessibility needs whom might find difficulty completing this form. For those without accessibility needs, our receptionist may direct callers to complete this online form.

Please do not use this form for any medical emergency. If you you a seriously unwell and feel it is an emergency, dial 999. If you are unwell and feel you cannot wait for a response during our opening hours, please call NHS 111.

Common queries

If you have a medication query, please submit an admin request online. Click 'Admin query' then click 'Something else' on the next page. Please provide as much detail as possible.

If you have a test result query, please submit an admin request online. You can still call the practice to find out your results, however, we may not have received the results yet.

If you need to change or cancel your appointment, please submit an admin request online. If you are unable to submit an online request, please call our reception team.

When you'll hear from us

All online forms received before 12pm Monday to Friday will be reviewed by a member of our clinical team the same day.  You will be contacted to arrange an appropriate follow up depending on the nature of your request.

If you submit your request after 12pm, this will be reviewed the next working day. Clinical queries are closed after 12pm on Friday to prevent an urgent query being delayed over the weekend.

Disabled Access & Facilities
